Care Management Temporary Staffing Agency, Inc. is seeking to hire experienced multilingual and multicultural R.N., LPN, CNA/HHA, Medical Assistants and Human Service Social Workers for part-time/full-time assignments in the Boston, North Shore, South Shore, and Metro West MA areas. Seeking the following positions:

R.N., LPN, CNA/HHA, Medical Assistants, and Human Service Social Workers

Care Management Temporary Staffing Agency, Inc. is a privately owned, fully insured home care staffing agency dedicated to providing outstanding case management care to our client’s consumers as they remain in the comfort of their own homes.

Nurses:

Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge in a home care environment. Acts as the liaison between patient/family and home care personnel to ensure necessary care is provided promptly and effectively. Must be RN with 2-4 years of clinical experience. Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a head of unit/department.

Human Service Social Workers:

Provides social-work services to patients and their families. Evaluates patients and their families to ensure proper social interaction and living environment. Require a bachelor’s degree in social work, sociology, psychology or other areas of specialty and 2-4 years of experience in the field or in a related area. Familiar with standard concepts, practices, and procedures within a particular field. Relies on limited experience and judgment to plan and accomplish goals. Performs a variety of tasks. Works under general supervision. A certain degree of creativity and latitude is required. Typically reports to a supervisor or manager.

Licensed Practical Nurses-LPN

Administers nursing care in a home care environment under the supervision of RN. Participates in the implementation and evaluation of patient care. Ensures the health, comfort and safety of patients. Requires a high school diploma and certification as a licensed practical nurse. Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Works under general supervision. A certain degree of creativity and latitude is required. Typically reports to RN or supervisor.

Certified Nurse Assistant/Home Health Aides:

Provides and supports patients with personal care. Assists and trains patient/client and family in activities of daily living such as bathing, grooming, toileting and elimination, and adequate nutritional intake. Requires a high school diploma or its equivalent and 0-2 years of experience in the field or in a related area. May be expected to meet certain state certifications, and be CPR certified. Familiar with standard concepts, practices, and procedures within a particular field. Relies on limited experience and judgment to plan and accomplish goals. Performs a variety of tasks. Typically reports to RN.

Responsibilities:

• Provide in-home managed care services for our client’s consumers in an accurate, disciplined and timely manner
• Evaluate the client’s clinical/non-clinical status and participate in the development of the total plan of care
• Initiate, follow-up and instruct other personnel in certain phases of plan of care
• Prepare and submit clinical/non-clinical notes and progress summaries based on the attainment of goals (Typically reports to a head of unit/department)
• Adhere to all universal pre-cautions
• Ensure the safety and wellbeing of consumers

Qualifications:

• Possess a graduate degree/certificate from an accredited school, university
• Must hold a valid license/certificate to practice in the Commonwealth of Massachusetts
• One to four years of appropriate experience preferred
• Home care /managed care experience preferred (not required)
• If driving, must be a licensed driver with an automobile that is insured in accordance with state requirements
• Ability to work as a self-starter or on a team
• Must be able to read, write and speak English proficiently. Speak other languages desired
